This form was developed to provide a way to communicate the family member’s mental health history

pursuant to the provisions of §5150.5 of the Welfare and Institutions Code, chaptered into law October 4,

2001 as Assembly Bill 1424. AB‐1424 modifies the LPS Act (Lanterman Petris Short Act) which governs

involuntary treatment for people with mental illness in California.
